These aren't hypotheticals — they're the most common things I see when I walk into a new client engagement.

5. No Review Gate

This is especially true for agentic AI systems. Agents are capable. They're also confidently wrong in ways that are hard to catch without a deliberate review layer. We've seen production deployments where an agent was doing exactly what it was told — and what it was told was subtly broken.

Build in the gate. Every AI output — whether it's generated content, automated decisions, or agentic code — needs a human-in-the-loop checkpoint until you've validated that the system is trustworthy in that domain.
